The Registered Nurse Case Manager serves as a vital link between clinical expertise and community-based care, specializing in hospice and home health services. This role is designed for a professional nurse who leverages community resources to plan and direct comprehensive care for individuals and families within their own homes. The position emphasizes community health education, ensuring that patients and their caregivers receive support that fosters self-sufficiency and comfort.
In this capacity, the RN Case Manager acts as the primary coordinator for an assigned caseload, managing the continuum of care from admission through ongoing treatment. The role requires a deep commitment to the nursing process, including conducting comprehensive assessments, developing personalized care plans with palliative actions, and continuously re-evaluating patient status. The Case Manager works in concert with an interdisciplinary team, physicians, and family members to address not only physical needs but also the emotional and educational requirements of the patient and their support system.
A typical day involves traveling to patient homes to perform detailed physical assessments and history reviews. The nurse initiates and revises plans of care based on real-time changes in patient condition, administering medications and treatments as prescribed. Beyond clinical duties, the role includes significant time spent counseling patients and families on self-care techniques, medication management, and diet. The Case Manager also maintains rigorous documentation in the EMR system, communicates critical updates to physicians, and coordinates with ancillary personnel to ensure equipment and services are available.
