The RN Case Manager Liaison Nurse serves as a critical bridge within the Kaiser Foundation Health Plan of Washington (KFHPW) continuum of care. Based at St. Peter Hospital in Olympia, this onsite position focuses on inpatient discharge planning and follow-up care for hospitalized patients. The role exists to ensure optimal patient health outcomes while maintaining the appropriate utilization of healthcare resources.
You will operate on a rotating schedule, working across two settings: planning discharges for patients at nearby network facilities and managing a caseload within a KFHPW medical center. This position requires a variable Monday-to-Friday schedule (10 PM to 10:30 AM) with an every-other-weekend rotation and alternating holidays. As the Liaison Nurse, you will establish collaborative care plans with primary and specialty care teams to improve functional status and patient self-management capabilities. You will also serve as the primary point of contact between KFHPW and external contracted providers, facilities, and resources, providing vital feedback on service quality and care delivery.
Sign-On Bonus: A $5,000 sign-on bonus applies to eligible external hires.
Shift Differential: Night shift differential pay applies to this position.
Requirements: Candidates must have prior Emergency Room (ER) or Case Management work experience. A Washington Registered Nurse license (or Compact License) is required at the time of hire, along with Basic Life Support certification. A Case Manager Certificate is required within 36 months of hire.
