As an RN Case Manager at Medical City Dallas, you will raise the bar by providing clinical expertise and the highest quality care in the most compassionate way. This role is designed for those who are continuous learners and wish to deliver excellent care to the patients who deserve it. You will partner with a multidisciplinary team to promote patient-centered care, coordinating the plan of care for the patient stay and managing the length of stay to ensure appropriate resource utilization.
Your day will involve facilitating the progression and transition of care using established criteria, ensuring a safe and appropriate discharge plan. You will act as a vital liaison between physicians, hospital staff, and outside agencies, advocating for patients' rights, needs, and values while considering their ethnic, cultural, or religious beliefs. This position offers a unique opportunity to support the under-resourced patient population and drive quality outcomes through effective care transitions.
