About the Role
TravelNurseSource is working with All Medical Personnel to find a qualified Case Manager RN in Washington. In this role, you will coordinate medical care, matching patient needs and preferences with the judicious use of healthcare resources. You will integrate, facilitate, and coordinate patient care for high-risk patients across the healthcare continuum, ensuring delivery of cost-effective, outcome-oriented, and quality care within an appropriate length of stay.
You will work in collaborative practice with physicians and other members of the healthcare team to actively support the delivery of quality care that meets patient-specific, age, and cultural needs. A key part of your day involves educating physicians regarding InterQual criteria, documentation of patient acuity, treatment plans, and the related expectations of CMS, JCAHO, NYS DOH, and other regulatory bodies. Your goal is to manage patient care to prevent fragmentation and duplication of services while linking cost and quality to patient care.

Requirements
- verified2-3 years Previous Case Management experience in an Inpatient/Acute Care Hospital Setting
