As a dedicated Case Manager at Highmark Inc., you will serve as the single point of contact for members, coordinating their care needs across various service delivery systems and community supports. This full-time, community-based position is designed for individuals who thrive in dynamic environments, requiring frequent travel within Delaware to visit members in their homes, nursing facilities, and other community settings.
You will work directly with members enrolled in DSHP Plus LTSS and DSNP programs, conducting face-to-face needs assessments and providing essential case management services. Your day will involve collaborating with integrated care teams to ensure members receive the safest, least restrictive, and most cost-effective care possible. From facilitating transitions between hospital and home to developing individualized care plans, you will play a pivotal role in helping members overcome obstacles and achieve their health and wellness goals.

Education: Bachelor's degree in Social Work or related field with 3 years experience; Master's degree in Social Work or related field with 1 year experience; High school degree or equivalent with 3 years case management experience.
One year in home clinical or case management experience. Certified Case Manager (CCM) certification. Licensed Bachelors Social Worker (LBSW), Licensed Masters Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW) licensure. Experience working with HIV/AIDS, behavioral health, or developmental disabilities populations. Medicare and Medicaid experience. Managed care experience. Experience in geriatric special needs, behavioral health, or home health. Experience with electronic documentation systems. Experience with cost neutrality and budgeting.
