As a Home Therapies RN CAP 1, you will serve as an onsite Patient Educator and Case Manager within the End Stage Kidney Disease (ESKD) healthcare team. This entry-level designation into the Clinical Advancement Program (CAP) is designed for Registered Nurses with less than two years of nephrology experience who are eager to specialize in home dialysis.
Your primary mission is to empower, engage, and train dialysis patients on peritoneal and home hemodialysis using state-of-the-art training programs and equipment. You will guide patients and their families through the transition to home therapy, assessing their learning needs, identifying barriers in the home environment, and ensuring they are ready to perform treatments safely. By collaborating with the interdisciplinary team, you will participate in decision-making and quality improvement activities that enhance patient care outcomes and facility operations.
Your work is primarily conducted in the clinic setting, though you will also conduct initial home visits alongside another staff member to evaluate home environments and partner readiness. You will manage a diverse group of patients across various shifts, including nights, weekends, and holidays, as well as participate in an on-call rotation.
Your day involves a blend of clinical care and education. You will assess patients' responses to home dialysis, manage treatment plans according to protocols, and administer medications as prescribed. A significant portion of your time is dedicated to training patients and home partners on the operation and maintenance of dialysis equipment. You will also serve as a resource for the healthcare team, assisting in the orientation of new staff and participating in the identification and education of future home dialysis candidates.
Candidates must be graduates of an accredited School of Nursing with a current state license and BLS certification. While entry-level nephrology experience is the standard for this CAP 1 role, a minimum of one year of RN experience and preferred home dialysis therapy experience are valued. Successful completion of Fresenius Kidney Care education and training requirements is mandatory for advancement within the program.
