As a Home Therapies RN CAP 1, you will serve as an entry-level professional within the Clinical Advancement Program (CAP), dedicated to the provision and coordination of clinically competent care for patients with End Stage Kidney Disease (ESKD). This position is pivotal in managing the patient journey from initial training through ongoing home dialysis therapy. You will act as a key member of the interdisciplinary healthcare team, participating in decision-making, leadership, and quality improvement activities designed to enhance patient outcomes and facility operations.
Your day-to-day work involves assessing and managing patient responses to home dialysis treatments, ensuring safe and effective delivery of therapy in the patient's home environment. You will collaborate closely with patients, families, and home care partners to identify learning needs, provide essential education, and evaluate readiness for home dialysis. This role requires a balance of clinical expertise and compassionate communication, as you guide patients and their support systems through the complexities of home treatment while maintaining strict adherence to prescribed protocols and safety standards.
