As a Home Therapies RN CAP 1, you will serve as an entry-level professional within our Clinical Advancement Program, dedicated to empowering and engaging dialysis patients. Your primary mission is to provide one-on-one training and education on peritoneal and home hemodialysis, utilizing state-of-the-art programs and equipment to ensure safe, effective care.
You will be a vital member of the End Stage Kidney Disease (ESKD) healthcare team, collaborating with physicians, educators, and the interdisciplinary team to enhance patient outcomes. This role involves assessing patient responses to home dialysis, managing treatment plans, and identifying barriers to home therapy. You will play a key part in the decision-making process regarding transplant and treatment modality options, ensuring patients and their families are fully informed and prepared.
Your work is primarily centered in the clinic at our Milwaukee location, with the added responsibility of conducting initial home visits alongside a staff member. You will perform systematic data collection and analysis, documenting patient progress and making necessary adjustments to treatment plans. A significant portion of your day is dedicated to education, where you will assess learning needs and train both patients and their home care partners on the operation and maintenance of dialysis equipment.
You will also assist with emergency response measures, coordinate laboratory procedures, and serve as a resource for the broader healthcare team. As you gain experience, you may take on leadership functions, supervising LPNs, LVNs, and Patient Care Technicians, while participating in staff orientation and quality improvement activities.
Candidates must be graduates of an accredited School of Nursing with current state licensure and BLS certification. This position is designed for Registered Nurses with less than two years of nephrology experience, though one year of general RN experience and home dialysis therapy experience are preferred. Successful completion of Fresenius Kidney Care education and training requirements is mandatory for advancement within the Clinical Advancement Program.
