Oak Street Health is dedicated to transforming healthcare for older adults in underserved communities. Our mission is to provide comprehensive, patient-centered care that improves quality of life and reduces hospitalizations through innovative care models and data-driven insights. We foster a supportive, inclusive culture where clinicians are empowered to make a meaningful impact on community health every day.
As a Primary Care Physician, you will manage a dedicated patient panel of adults on Medicare, focusing on prevention, chronic disease management, and acute care. This role exists to close care gaps and enhance outcomes for a vulnerable population by leveraging evidence-based guidelines and population health data. You will collaborate closely with an interdisciplinary team—including nurses, behavioral health specialists, and community health workers—to coordinate seamless care and ensure continuity with specialists and community resources.
Your day-to-day involves conducting detailed history and physical examinations, developing individualized treatment plans, and engaging patients and families in shared decision-making. Beyond clinical duties, you will participate in team huddles, case reviews, and quality improvement initiatives to continuously enhance the patient experience. We offer mentorship, leadership development, and growth opportunities to help you thrive while upholding our values of inclusive and collaborative care.
Interested candidates should submit their application through our careers portal. The interview process typically includes an initial screening to discuss your background and fit, followed by a clinical assessment or case review, and concludes with a team interview to ensure alignment with our culture and values.
