As a Physician - Primary Care, you will serve as a Primary Care Provider (PCP) at our Community Based Outpatient Clinic (CBOC). Operating under the Patient Aligned Care Team Model, you will work collaboratively with a dedicated team including RNs, LVNs, and Clerks to care for a panel of patients.
In this team-based setting, you will manage a wide variety of chronic conditions and preventive health needs, including diabetes, hypertension, COPD, degenerative arthritis, depression, congestive heart failure, and chronic pain. You will evaluate, diagnose, and treat non-surgical adult patients of all ages, while also assessing and stabilizing emergent conditions. Additionally, you will collaborate with and oversee mid-level providers and work with various specialties both within and outside the VA system to ensure comprehensive patient care.
