As a Primary Care Physician, you will deliver comprehensive, patient-centered care to a panel of adult patients, with a specific focus on those who have historically faced barriers within the healthcare system. This role exists to close care gaps and enhance outcomes for diverse, often vulnerable populations through longer appointment times and smaller patient panels. You will engage patients in their care, manage acute and chronic conditions such as diabetes and hypertension, and utilize population health data to address both medical and social determinants of health.
Your work involves conducting thorough evaluations, managing complex care plans, and collaborating closely with an interdisciplinary team including nurses, social workers, and behavioral health specialists. You will participate in routine follow-ups, annual wellness visits, and transitional care management, while reviewing diagnostic results and documenting encounters in the electronic health record. Beyond clinical duties, you will contribute to quality improvement initiatives, monitor key performance metrics like HEDIS measures, and engage in continuous learning through case conferences and team huddles.
