About the Role
As a Primary Care Provider (PCP), you will serve at a Community Based Outpatient Clinic (CBOC) operating under the Patient Aligned Care Team Model. You will work collaboratively with a dedicated team—including RNs, LVNs, and Clerks—to care for a panel of patients. Your day-to-day involves providing comprehensive primary care services in a team-based setting, focusing on chronic disease management and preventive health. You will encounter a wide variety of conditions, including diabetes, hypertension, COPD, degenerative arthritis, depression, congestive heart failure, and chronic pain. Additionally, you will assess and stabilize patients with emergent conditions and collaborate with mid-level providers and specialists both within and outside the VA system to manage patient health.
