As a Primary Care Physician, you will provide comprehensive, patient-centered care focused on prevention, chronic disease management, and improving overall health outcomes. You will build long-term relationships with a panel of adult and senior patients, conducting thorough evaluations and managing acute and chronic conditions such as diabetes, hypertension, COPD, and heart disease.
This role delivers high-quality outpatient primary care through in-person visits and telehealth encounters. You will utilize evidence-based guidelines, population health tools, and data analytics to identify care gaps, reduce avoidable hospitalizations, and promote proactive care. Your day will involve coordinating with a multidisciplinary team—including nurses, medical assistants, care managers, behavioral health professionals, and community health workers—to design and refine care models that enhance accessibility.
You will actively participate in team huddles, case conferences, and quality improvement initiatives. A key part of your mission is engaging families and caregivers, communicating empathetically with diverse populations, and advocating for resources that address social determinants of health. While maintaining a manageable schedule to support unhurried patient visits, you will drive performance on quality metrics and patient satisfaction.
