This entry-level Master of Social Work (MSW) position is designed to provide essential psychosocial services to patients undergoing dialysis treatment, both in-center and at home. The role serves as a vital bridge between clinical care and the emotional, psychological, and behavioral realities of living with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD).
As a member of the interdisciplinary team, you will work to promote positive adjustment, rehabilitation, and an improved quality of life for our patients. Your mission involves utilizing social work theory and accepted practice methods to assess patient needs, facilitate care planning, and connect individuals with community resources. You will actively support the organization's commitment to quality indicators and outcomes, ensuring that every patient receives comprehensive, compassionate care that addresses barriers to treatment adherence and overall well-being.
We are dedicated to the FMCNA commitment to Quality Indicators and Outcomes (QAI). In this role, you will actively participate in process improvement activities that enhance the likelihood of patients achieving their quality goals. This includes monitoring psychosocial status, addressing non-adherence, and participating in monthly quality review meetings.
We foster a collaborative environment where social workers inform, educate, and support staff in understanding the profound impact of kidney disease on patients and their families. By maintaining current knowledge of local vocational and educational programs, and by facilitating discussions on advance directives and DNR status, you help ensure that care is not only medically sound but also deeply human-centered.
