About the Role
This entry-level Master of Social Work (MSW) position is designed to provide essential psychosocial services to patients receiving in-center and home dialysis care. The role serves as a vital bridge between clinical treatment and the emotional, psychological, and behavioral realities of living with Chronic Kidney Disease (CKD). By utilizing social work theory and accepted practice methods, the Social Worker promotes positive adjustment, rehabilitation, and an improved quality of life for patients and their families.
The position covers three clinics: South Fort Myers, Punta Gorda, and Naples. As a member of the interdisciplinary team, you will collaborate with physicians, nurses, and administrative staff to ensure comprehensive, high-quality care. Your work directly supports FMCNA's commitment to Quality Indicators and Outcomes, actively participating in process improvement activities that help patients achieve their health goals.
Day in the Life
Your day involves assessing patients' psychosocial status, strengths, and areas of need to develop effective care plans. You will provide supportive counseling to help patients adjust to dialysis, offer goal-directed counseling for those seeking transplants, and facilitate referrals to community resources such as home health services and vocational rehabilitation. A significant portion of your time will be dedicated to patient education, addressing barriers to treatment adherence, and explaining complex topics like advance directives and DNR status in collaboration with the medical team.
You will also act as a liaison between patients and insurance coordinators to navigate payment issues, entitlement programs, and billing solutions. Additionally, you will contribute to monthly quality review meetings, report on key indicators like missed treatments and quality of life trends, and assist in training staff on psychosocial topics. The role requires adherence to state regulatory caseload guidelines and may involve travel between the three covered facilities.
Hiring Process
This is an entry-level opportunity for candidates with 0–2 years of related experience. Applicants must hold a Master's degree in Social Work and possess a state temporary license, Registered Social Worker Clinical Intern, Associated Social Work license, or Provisional LCSW License (as required by the state). Successful candidates are expected to obtain necessary state licensure to work independently within the first two years.
