About the Role
This position is designed to deliver exceptional physical therapy care in a home health setting. You will serve as a vital link between clinical expertise and patient well-being, focusing on relieving pain and restoring functional performance. Your day-to-day involves conducting comprehensive assessments, developing treatment plans in collaboration with physicians, and utilizing physical means such as exercise, massage, and therapeutic modalities to aid recovery. Beyond direct patient care, you will play a mentorship role by teaching and counseling families on care routines and the proper use of assistive devices like wheelchairs and braces. You will also provide essential oversight to Physical Therapy Assistants and paraprofessionals, ensuring the highest standards of care are maintained across all visits.

Key Responsibilities
