- check_circleObserve and report patient reactions, changes, and progress to attending physicians and agency staff
- check_circleTreat patients to relieve pain and restore function using exercises, massage, and other physical means
- check_circlePerform OASIS assessments to develop and adjust individualized plans of care
- check_circleProvide skilled physical therapy services to patients in home settings as ordered by physicians
- check_circleComplete accurate clinical documentation following agency protocols and Medicare guidelines
- check_circleEducate patients and families on disease management, preventative measures, and proper equipment use
Requirements
- verifiedPhysical Therapist with a current license in the state of employment
- verifiedMinimum one-year experience as a PT in an acute care setting
