About the Role
We are seeking Full-Time and PRN Physical Therapists to join our team in the Lake City, Lake Butler, Columbia, Hamilton, Suwanee, and Lafayette areas. In this role, you will partner with an interdisciplinary care team to optimize patient outcomes and help individuals heal and regain their independence. You will be responsible for performing assessments, developing tailored treatment plans, and providing education to patients and their families. Key Responsibilities

- check_circleDevelop and implement tailored treatment plans in consultation with physicians and clinicians
- check_circleCollaborate with the interdisciplinary care team to optimize patient outcomes
- check_circleProvide education and support to patients and families on therapy programs
- check_circleMaintain accurate clinical records and report changes in patient condition to physicians
- check_circleSupervise Physical Therapy Assistants in accordance with agency policies and state regulations
- check_circleParticipate in in-services, case conferences, and meetings to promote coordinated care
- check_circleIdentify patient and family needs for additional services and make appropriate referrals
- check_circleConfirm daily visit schedules to ensure seamless coordination with other staff
- check_circleDocument all visits and progress notes on the same day of service
Requirements
- verifiedCurrent PT license (state-specific)
- verifiedGraduate of an accredited physical therapy program
- verifiedValid BLS certification
- verifiedMinimum one year of PT experience
- verifiedValid driver's license and access to reliable insured vehicle
- verifiedProficiency with electronic medical records
