About the Role
We are seeking a dedicated Physical Therapist to join our team. In this position, you will be instrumental in evaluating and treating patients across a variety of conditions, ranging from orthopedic and neurological cases to post-surgical and medically complex needs. Your primary mission is to develop individualized care plans that restore function and significantly improve the quality of life for those under your care.
As a member of our multidisciplinary team, you will collaborate closely with physicians, nurses, and case managers to coordinate and optimize patient care. Your day will involve providing therapeutic interventions, educating patients and families on home exercise programs and safety strategies, and ensuring that every interaction is grounded in evidence-based, patient-centered principles. Key Responsibilities
- check_circlePerform comprehensive physical therapy evaluations for inpatient and outpatient populations
- check_circleDevelop and implement individualized, evidence-based plans of care with measurable functional goals
- check_circleProvide therapeutic interventions including exercise, gait training, and manual therapy
- check_circleEducate patients and families on home exercise programs and safety precautions
- check_circleRecommend, fit, and train patients in the use of assistive devices and durable medical equipment
- check_circleCollaborate with physicians and nurses to coordinate and optimize patient care
- check_circleDocument evaluations, treatment notes, and progress reports accurately in the EMR system
- check_circleMonitor patient progress and update treatment plans and goals accordingly
- check_circleContribute to safe and effective discharge planning and follow-up services
- check_circleParticipate in quality improvement, patient safety initiatives, and department meetings
Requirements
- verifiedCurrent Illinois state license (or eligibility) as a Physical Therapist
- verifiedDoctor of Physical Therapy (DPT) from an accredited program
- verifiedBasic Life Support (BLS) certification
- verifiedExperience with electronic medical records (EMR) systems
Nice to Have
Advanced Cardiovascular Life Support (ACLS) certification. Certification in a specialty area (e.g., Orthopedics, Neurology, Geriatrics). Experience with electronic medical records (EMR) systems.
