You'll provide home-based therapy care for patients as directed by the attending physician, interacting with patients' families while experiencing the rewarding privilege of being part of every step of their recovery journey. How You'll Work
You'll provide comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members and ensure the safety of the patient. You are accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.

Key Responsibilities
- check_circleSupervise therapy services provided by PTAs and nurse assistants and communicate supervisory results
- check_circleProvide skilled physical therapy services in patient homes as ordered by attending physicians
- check_circleEducate patients and families on disease management, preventative measures, and proper equipment use
- check_circleConduct initial evaluations and develop individualized plans of care within 48-72 hours of referral
- check_circlePerform OASIS assessments to monitor patient progress and adjust treatment plans accordingly
- check_circleComplete all clinical documentation, including assessments and treatment notes, in compliance with Medicare guidelines
- check_circleCoordinate admissions, transfers, and discharges to ensure continuity of care and reduce re-hospitalizations
- check_circleParticipate in case conferences, on-call requirements, and in-service training sessions
Requirements
- verifiedPhysical Therapist with a current license in the state of employment
- verifiedMinimum of one year of experience as an PT in an acute setting
- verifiedValid driver's license and auto insurance with your name as a listed driver
- verifiedCurrent negative tuberculin skin test or x-ray or TB testing at orientation
