About the Role
We are currently seeking a Physical Therapist to join our team in the Omaha Metro area. This full-time position requires Monday through Friday visits. In this role, you will be instrumental in evaluating patient mobility, coordination, and strength, while developing comprehensive plans of care that define goals and expected outcomes. You will serve as a resource consultant for clinical staff, collaborating with physicians, nurses, and families to coordinate services and ensure effective communication. Additionally, you may supervise other personnel, such as PTAs and HHAs, and complete necessary OASIS assessments in compliance with state regulations.
Hiring Process
Candidates must hold a degree in Physical Therapy from an accredited program, possess a current unrestricted state license, and maintain a valid driver's license and CPR certification. As an employer accepting Medicare and Medicaid funds, all employees must comply with relevant health-related requirements, including vaccinations and testing, subject to applicable exemptions.

- check_circleMaintain plans of care through timely documentation, goal modification, and discharge planning
- check_circleCollaborate with healthcare teams, physicians, and families to coordinate patient services

Requirements
