CHI Health at Home is part of an expansive healthcare network committed to providing better patient care and outcomes where it is best received: in the home. As a faith-based organization, we are rooted in humankindness, with our ministry at the heart of everything we do. We strive to find new ways to improve the health of our patients, their families, and the communities we serve.
We are seeking a skilled and compassionate Physical Therapist to join our team. The focus of Physical Therapy is to improve functional movement, and practicing in the home setting—where patients spend most of their time—is the most functional place to make an impact. In this role, you will work closely with patients, families, caregivers, and other healthcare professionals to develop and implement personalized treatment plans that address each patient's unique needs and goals. Your work will promote rehabilitation, recovery, and an improved quality of life for those you serve.
At CommonSpirit Health at Home, we are proud to be an Equal Opportunity Employer, promoting diversity, equity, and inclusion in every aspect of our organization. We value the unique contributions of all individuals, including minorities, protected veterans, and individuals with disabilities.
