About the Role
We are seeking a dedicated Occupational Therapist to join our team in Sioux Falls and surrounding cities. In this full-time, Monday-through-Friday role, you will utilize your clinical expertise to assess, teach, and deliver therapy interventions directed by physicians within the home health plan of care. You will collaborate closely with an interdisciplinary team, exchanging pertinent updates to optimize patient well-being and serve as a resource consultant for clinical staff.

Key Responsibilities
- check_circlePerform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function
- check_circleProvide written evaluations and plans of care defining occupational therapy needs and expected outcomes
- check_circleDeliver occupational therapy services in compliance with prescribed physician orders
- check_circleMaintain plans of care by documenting client performance, modifying goals, and determining discharge plans
- check_circleAdminister, evaluate, and interpret tests to determine functional goals and outcomes
- check_circleCoordinate services with healthcare teams, physicians, nurses, and family members
- check_circleConduct discharge planning and educate patients and caretakers on home exercise plans
- check_circleComplete OASIS assessments in compliance with state professional practice and regulations
- check_circleSupervise other personnel such as OTAs and HHAs as applicable
Requirements
- verifiedDegree in Occupational Therapy from an accredited university/program
- verifiedCurrent, unrestricted state license as an Occupational Therapist
- verifiedValid CPR
- verifiedValid Drivers License
- verifiedProof of Automobile Insurance
