About the Role
In this in-home care role, you will assess, plan, and deliver individualized therapy for clients with complex medical needs, including former energy workers and their families. You will partner with an interdisciplinary team of nurses, physicians, and case managers to maximize patient function, safety, and independence.
Your day will involve traveling to patient homes to perform comprehensive functional assessments and provide ongoing treatments focused on improving mobility, function, and quality of life. You will educate patients and family caregivers on home exercise programs, safe transfer techniques, and fall-prevention strategies, while recommending and training patients in the use of assistive devices to promote safety.

Requirements
- verifiedCurrent state licensure as a Physical Therapist (PT)

- verifiedValid driver's license and reliable transportation
