About the Role
In this in-home care role, you will assess, plan, and deliver individualized therapy for clients with complex medical needs, including former energy workers and their families. You will partner with an interdisciplinary team of nurses, physicians, and case managers to maximize patient function, safety, and independence. This position offers the opportunity to make a meaningful impact by improving mobility, function, and quality of life for those who need it most.

Requirements
