About the Role
In this in-home care role, you will assess, plan, and deliver individualized therapy for clients with complex medical needs, including former energy workers and their families. You will partner with an interdisciplinary team of nurses, physicians, and case managers to maximize patient function, safety, and independence.
Your day will involve traveling to patient homes to perform comprehensive functional assessments, develop evidence-based treatment plans, and educate patients and caregivers on safe transfer techniques and fall-prevention strategies. You will continuously reassess progress and modify care plans to ensure the highest quality of life for those you serve.

Requirements
- verifiedCurrent state licensure as a Physical Therapist (PT)
