Key Responsibilities
- check_circlePerform daily reactive and proactive medication regimen reviews for patients
- check_circlePrecept pharmacy students and residents on clinical patient care rotations
- check_circleProvide patient-specific and non-patient specific drug information support
- check_circleParticipate in quality management initiatives including medication use evaluations and error reporting
- check_circlePresent in-services and develop educational materials for pharmacy, medical, and nursing staff
- check_circleParticipate in research projects and submit scholarly work for publication
- check_circleCollaborate with healthcare professionals to manage patient drug therapy and minimize adverse reactions
- check_circleSupport delivery of pharmacy managed protocols, formulary processes, and medication reconciliation
- check_circleConduct proactive drug therapy reviews, monitoring, and patient counseling
- check_circleContribute to the design, analysis, and reporting of quality improvement program findings
Requirements
- verifiedGraduate of education program approved by credentialing body
- verifiedASHP-accredited PGY1 residency or equivalent (3 years pharmacy experience per residency year) OR 3 years clinical practice experience
- verifiedASHP-Qualified Residency Preceptor within one year
- verifiedTexas Pharmacist License or eligible
- verifiedPPC- Pharmacist Preceptor Cert
- verifiedBLS (AHA)
- verifiedACLS (AHA)
Nice to Have
ASHP-accredited post-graduate year two (PGY2) residency or fellowship experience. PALS (Pediatric Advanced Life Support) certification. IV Certification. Board of Pharmacy Specialty certification.
