The Clinical Pharmacist serves as a vital guardian of patient safety by overseeing the medication distribution system and ensuring every prescription is indicated, effective, safe, and convenient. This role goes beyond dispensing; it involves actively identifying, preventing, and resolving drug therapy problems to optimize patient outcomes. You will interact with colleagues, visitors, and staff in alignment with Jefferson's core values, ensuring compliance with hospital policies and the Pharmacy & Therapeutics Committee's formulary.
In this position, you will monitor patients for their response to medications and document any sentinel events, near misses, or adverse drug reactions. A key part of your day involves training and precepting the next generation of pharmacists, including residents, interns, technicians, and student pharmacists. You will provide care tailored to the chronological age, developmental stage, and cultural background of every patient you serve.
