Community Healthcare Network (CHN) is a not-for-profit organization dedicated to providing more than 65,000 New Yorkers with primary and behavioral healthcare, dental, nutrition, wellness, and essential support services. Our network comprises 14 federally qualified health centers across Brooklyn, the Bronx, Queens, and Manhattan, complemented by a fleet of mobile vans that deliver health services directly to underserved populations throughout New York City.

This position serves as a vital bridge between our community and the healthcare services we provide. As a peer educator, you will be a member of the targeted communities and service groups, bringing a unique perspective and deep understanding to your work. You will conduct outreach in targeted areas, distribute HIV/AIDS information and resources, and facilitate connections between clients and necessary health and support services.
You will work within a supportive, interdisciplinary team where your ideas and contributions are valued.
