We are seeking a dedicated professional to join our Health Services Team. In this role, you will be an integral part of a group committed to providing comprehensive care to youth in our residential programs. Your work will directly impact the lives of young people by ensuring their medical needs are met with compassion and clinical expertise.
As a Nurse Case Manager, you will provide essential medical case management services to our residential programs. Your day-to-day responsibilities include screening, education, counseling, and medication management for the youth in our care. You will act as a vital link between our residents and the broader healthcare system by networking with community health care providers.
A key component of this position involves participating in a rotating on-call schedule. This primarily consists of remote phone consultations, ensuring that medical support is accessible to our residents without the need for constant on-site presence. We offer flexible scheduling policies, allowing employees to customize their work hours to better balance their professional and personal lives.
Cunningham Children's Home is located in Urbana, Illinois, part of the twin cities of Champaign-Urbana in east-central Illinois.
