About the Role
As a Nurse Case Manager, you will join a team committed to serving youth in critical need. This role is designed to provide comprehensive medical case management within our residential services programs. You will be responsible for screening, education, counseling, and medication management, while also networking with community healthcare providers to ensure seamless care.
The position offers a unique blend of clinical expertise and flexibility. You will participate in a rotating on-call schedule, primarily providing phone consultations to ensure accessibility without the need for constant on-site presence. Employees have the option to customize their work hours through our flexible scheduling policy, allowing you to balance professional responsibilities with personal needs.
