About the Role
As a Medical Social Worker (MSW), you will join an elite team of healthcare professionals dedicated to providing the Right Care, at the Right Time, in the Right Place. This role is about carrying out our mission every day through your true calling: developing an amazing team of compassionate and dedicated healthcare providers.
You will be a key part of our industry pioneer efforts, delivering unparalleled care by assessing patients to identify psychosocial, financial, and environmental needs. Your day will involve making initial social work evaluations, reevaluating needs during follow-up visits, and collaborating with supervisors, physicians, and other personnel to communicate significant findings and ensure patient safety. You will implement plans using patient, family, and community resources, and participate in the development of Plans of Care to ensure quality, continuity, and proper discharge planning.
