This position provides essential psychosocial services to patients receiving in-center and home dialysis care. As a vital member of the interdisciplinary healthcare team, you will utilize Social Work Theory of Human Behavior to promote positive adjustment, rehabilitation, and an improved quality of life for individuals living with Chronic Kidney Disease. Your work ensures comprehensive care by informing, educating, and supporting staff in understanding the emotional, psychological, and behavioral impacts of the disease on patients and their families.
You will actively support the organization's commitment to Quality Indicators and Outcomes (QAI) by participating in process improvement activities that enhance patient satisfaction and help patients achieve quality goals. This role involves a dynamic day-to-day rhythm of assessing patient needs, facilitating care planning, and providing supportive counseling to help patients adjust to dialysis, navigate the transplant process, and access vital community resources.
