This entry-level Master of Social Work (MSW) position is designed to provide essential psychosocial services to patients undergoing dialysis treatment, both in-center and at home. The role serves as a vital link within the interdisciplinary healthcare team, dedicated to promoting positive adjustment, rehabilitation, and an improved quality of life for individuals living with Chronic Kidney Disease.
As a Social Worker at Fresenius Kidney Care, you will utilize established social work theories and accepted practice methods to support patients and their families through the emotional, psychological, and behavioral challenges of their condition. Your work directly contributes to the organization's commitment to Quality Indicators and Outcomes, ensuring comprehensive, high-quality care that addresses the unique needs of every patient.
You will collaborate closely with physicians, nurses, and other care team members to assess patients' psychosocial status, strengths, and areas of need. This involves conducting comprehensive assessments to identify barriers to optimal treatment outcomes and developing care plans that align with patient rehabilitation goals.
Your daily activities include providing supportive counseling, facilitating discussions on advance directives and DNR status, and assisting patients with referrals to community resources such as home health services and vocational rehabilitation. You will also play a key role in patient education, helping individuals and families understand their rights, treatment modalities, and the psychosocial aspects of End Stage Renal Disease (ESRD). Additionally, you will act as a liaison for insurance and financial assistance issues, ensuring patients have the support they need to navigate payment and coverage complexities.

This role actively participates in Quality Assessment and Improvement (QAI) activities, including monthly quality review meetings, to enhance patient satisfaction and adherence to treatment plans.
