This position provides essential psychosocial services to patients receiving in-center and home dialysis care. As a vital member of the interdisciplinary healthcare team, the Social Worker utilizes Social Work Theory of Human Behavior to promote positive adjustment, rehabilitation, and an improved quality of life for individuals living with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD).
The role exists to bridge the gap between medical treatment and the emotional, psychological, and behavioral realities of chronic illness. You will collaborate closely with physicians, nurses, and other staff to educate the team on the impact of kidney disease on patients and families, ensuring comprehensive, patient-centered care. Your work directly supports Fresenius Medical Care North America's (FMCNA) commitment to Quality Indicators and Outcomes, actively participating in process improvement activities to help patients achieve their health goals.
Your day involves assessing patients' psychosocial status, strengths, and areas of need to develop effective care plans. You will provide supportive counseling to help patients adjust to dialysis, offer goal-directed counseling for those seeking transplants, and facilitate referrals to community resources such as home health services and vocational rehabilitation. A significant portion of your time is dedicated to patient education, addressing barriers to treatment adherence, and guiding patients through complex topics like advance directives and DNR status discussions.
You will also serve as a liaison between patients and insurance coordinators to address payment issues and coverage changes. By maintaining current knowledge of local resources and adhering to FMCNA policies, you ensure that every patient receives the support needed to navigate their treatment journey with dignity and clarity.
