The Case Management department plays a pivotal role in this mission, ensuring that patient care is coordinated seamlessly across the continuum of treatment.
This leadership position manages the daily operations of the Care Coordination department, overseeing a diverse team of RN Case Managers, Social Workers, and administrative staff. The role exists to bridge the gap between clinical needs and operational efficiency, ensuring that nursing care is delivered based on evidence-based practices and the specific age-related and clinical needs of our patients.
In this role, you will foster a culture of continuous improvement by identifying process gaps and collaborating with team members to resolve barriers. You will serve as a vital communication link between leadership and frontline staff, ensuring that departmental goals, standards, and objectives align with the hospital's strategic vision. Beyond operations, you will be responsible for the growth and development of your team through coaching, mentoring, and managing performance, scheduling, and recruitment.
Candidates with an MSN or MBA, along with significant Case Management and Leadership experience, are preferred. The application process involves reviewing qualifications against the required professional licenses for the specific state of work location (Illinois, Missouri, Oklahoma, or Wisconsin), which include Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) credentials.
