This position serves as a vital link in the skilled nursing care continuum, providing professional nursing care, assessment, and evaluations to residents within the scope of practice. You will be part of a team that prioritizes resident well-being, ensuring care is implemented consistently and appropriately for the age of those served.
In this role, you will perform licensed practical nursing functions under the direction of the Registered Nurse. Your day will involve completing resident assessments, making appropriate interventions, and utilizing the nursing process (assess, plan, implement, evaluate) to prioritize care. You will manage the delivery of resident care, ensuring quality and safety in medication administration and treatment according to physician orders. Additionally, you will participate in care planning, guide staff in meeting resident needs, and communicate changes in resident conditions to physicians using the SBAR tool.
This role is ideal for new graduates or those with less than one year of experience in Long Term Care. You will have the opportunity to seek educational growth, share knowledge with colleagues, and assist in the orientation of new staff.
This is a regular position featuring varied shifts. The schedule includes Monday through Friday rotations covering days, evenings, nights, weekends, and holidays. Specific shifts run from 7:00 am to 7:30 pm or 7:00 pm to 7:30 am, with a pattern of every third weekend and holiday. The position requires a commitment of 48 hours every two weeks.
