As a Licensed Practical Nurse (LPN) in our Primary Care patient services, you will be a valued member of an interprofessional care team. You will partner with physicians, advanced practitioners, registered nurses, and allied health staff to deliver patient-centered care across a spectrum of needs, including preventative care, acute care, and chronic condition management.
Care is provided through in-person visits, phone, video, and web-based interactions. You will play a vital role in care across specialties, managing patient communications, administering treatments such as vaccines and wound care, and supporting preventative initiatives. The environment is fast-paced and team-oriented, allowing you to work closely with an interdisciplinary team while building meaningful, long-term relationships with patients.
