About the Role
CareRite Centers seeks a compassionate Licensed Practical Nurse to provide resident-centered care within our skilled nursing and rehabilitation facility. In this role, you will deliver direct bedside nursing, administer medications and treatments, and monitor resident conditions while documenting care in EMR systems. You will collaborate closely with an interdisciplinary team of RNs, therapists, and physicians to implement individualized care plans that support both rehabilitation and long-term care goals.
Beyond clinical duties, you will play a vital part in educating residents and families on care plans, medications, and health management. Your work will directly contribute to promoting dignity, safety, and comfort for every resident.
