The Hospice Registered Nurse plays a vital role in delivering compassionate care to terminally ill patients and their families in Jefferson County. Operating within the established plan of care, this position focuses on pain management and symptom control, ensuring a high quality of life for patients during their final journey. The role serves as a central coordinator between the patient, family, interdisciplinary team, attending physician, and Hospice Medical Director.
Your day involves assessing the total needs of patients and families during scheduled and after-hours home visits, documenting interventions, and reviewing the effectiveness of care plans. You will collaborate closely with the interdisciplinary team to develop and revise clinical components of patient care, ensuring continuity between home care, long-term facilities, and inpatient settings. A significant portion of the role includes supervising Hospice Aides and Licensed Practical Nurses, facilitating care in long-term care facilities, and providing education to families on the hospice concept of care.
The position requires active participation in an on-call rotation for evenings, weekends, and holidays to ensure timely delivery of services. You will also manage the certification and recertification of patients, monitor the appropriateness of hospice services, and maintain strict compliance with Medicare Conditions of Participation, Joint Commission standards, and agency policies.
This full-time role is based in Jefferson County with a Monday through Friday schedule, including an on-call rotation for after-hours coverage.
