In this On Call Registered Nurse (RN) position, you will lead with empathy during home visits, assessing patient and family needs to craft care strategies that prioritize comfort, dignity, and peace. You will empower families and caregivers through clear education on hospice services, symptom management, and available support resources. Additionally, you will guide and mentor your team, training Hospice Aides and LPNs to deliver safe, effective, and compassionate care at home.
You will collaborate seamlessly with physicians, interdisciplinary teams, and facility staff to create smooth transitions and coordinated care plans. As a trusted resource, you will provide hands-on expertise in pain control, medication administration, and symptom relief, ensuring patients and families receive timely support on weekends and holidays through on-call rotations.
