We are seeking an Outreach Where You Live (OWLS) Unsheltered Outreach Caseworker to join our new Unsheltered Outreach program serving the Waterbury and Litchfield County areas. In this role, you will oversee a variety of cases across different venues, providing unsheltered individuals with the necessary tools to accomplish their goals.
Your day will involve active street outreach, managing equipment and supplies, and coordinating with external support providers to further individuals' attempts to end their homelessness. You will operate within the program's model and adhere to protocols established by the Western CT Mental Health Network and the Coordinated Access Network. Success in this position requires establishing open communication, support, and accountability among collaborating agencies.
