About the Role

As a Case Manager, you will provide quality care by developing, implementing, managing, and evaluating patient care plans. You will serve as a vital link, conveying clinical information between insurance providers and the treatment team as warranted. This role is essential for coordinating the continuum of care, ensuring optimum utilization of resources, and maintaining compliance with external agencies and referral sources.

In this position, you will engage in a variety of duties related to coordinating treatment and discharge activities. You may facilitate linkages to community resources, assist patients in obtaining necessary services such as housing, financial aid, and medical care, and help patients build social support networks. You will also conduct individual and family educational sessions and work closely with the team to address issues relevant to the treatment milieu.
