About the Role

This position serves as a vital link between patients, physicians, and the broader care team, dedicated to delivering professional nursing care in accordance with established standards and practices. The role is designed to ensure comprehensive patient support through assessment, treatment planning, and continuous medical evaluation.

In this capacity, you will function as the primary liaison, coordinating care efforts to optimize patient outcomes. Your day-to-day involves obtaining detailed health histories, assessing disease conditions, and executing prescribed medical interventions. You will be responsible for administering medications, monitoring patient responses, and maintaining rigorous documentation within automated medical record systems.

Beyond direct clinical care, this role emphasizes patient education and advocacy. You will educate patients and their families on health maintenance and the management of both acute and chronic conditions. The position requires active participation in multidisciplinary teams to refine care processes and ensure a safe, compliant work environment. You will also handle patient triage, respond to treatment-related inquiries, and initiate crisis interventions when necessary.
