As a Home Health Physical Therapist, you will become a vital part of our caring community. Reporting to the Clinical Manager, your mission is to help patients regain strength, mobility, and independence from the comfort of their own homes. By delivering personalized care that focuses on functional improvement, you will empower individuals to overcome limitations and enjoy a better quality of life.
In this role, you will plan and administer prescribed physical therapy treatments onsite to attain the highest level of physical function. You will assess and document patients' physical strengths to support physician evaluations and guide treatment goals. Working collaboratively with a team of healthcare professionals, you will develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and the patient's goals. Additionally, you will provide clear written care instructions to Physical Therapy Assistants (PTAs) and Home Health Aides, overseeing their performance and documentation while ensuring all patient observations, interventions, and evaluations are documented promptly and thoroughly.
To join our team, candidates must hold a degree from an accredited Physical Therapy Program (approved by the APTA), possess a current and unrestricted Physical Therapy license, and maintain current CPR certification. Experience collaborating with a team of healthcare professionals is required, with one year of experience as a Physical Therapist preferred.
Please note that this role is patient-facing and part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to be screened for TB. Furthermore, as this role is part of Humana's driver safety program, you must have a valid state driver's license and maintain personal vehicle liability insurance in accordance with your residing state's minimum required limits (or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
