About the Role

As a Home Health Physical Therapist at CenterWell Home Health, you will join a caring community dedicated to helping patients regain strength, mobility, and independence from the comfort of their homes. Reporting to the Clinical Manager, you will deliver personalized care that focuses on functional improvement, empowering individuals to overcome limitations and enjoy a better quality of life. Your work will involve collaborating with a multidisciplinary team of nurses, occupational therapists, speech-language pathologists, home health aides, and medical social workers to ensure high-quality, accessible, and comprehensive care.

Hiring Process & Requirements

This role is patient-facing and part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to undergo TB screening. Additionally, as this position is part of Humana's driver safety program, you must hold a valid state driver's license and maintain personal vehicle liability insurance in accordance with your residing state's minimum required limits (or $25,000 bodily injury per person/event and $10,000 for property damage, whichever is higher).
