About the Role
As a Home Health Physical Therapist, you will become a vital part of our caring community. Reporting to the Clinical Manager, you will deliver personalized care directly in patients' homes, helping them regain strength, mobility, and independence. Your mission is to empower individuals to overcome limitations and enjoy a better quality of life by focusing on functional improvement. You will collaborate closely with physicians, the care team, and families to develop and implement rehabilitation programs that align with each patient's unique goals and the physician's Plan of Treatment.
Hiring Process & Requirements
This role is patient-facing and requires a current, unrestricted Physical Therapy license and current CPR certification. Candidates must have experience collaborating with a team of healthcare professionals. A valid state driver's license and personal vehicle liability insurance meeting state minimums (or specific coverage limits) are required, as this position is part of Humana's driver safety program. Selected candidates will also be required to undergo Tuberculosis (TB) screening as part of Humana's health safety protocols.
