As a Home Health Physical Therapist, you will work onsite in patients' homes to attain the highest level of physical function. You will plan and administer prescribed physical therapy treatments, assessing and documenting patients' physical strengths to support physician evaluations and guide treatment goals. Your role involves developing and implementing rehabilitation programs consistent with the physician's Plan of Treatment and the goals of the patient team. Additionally, you will provide clear written care instructions to Physical Therapist Assistants (PTAs) and Home Health Aides, overseeing their performance and documentation. You will serve as a vital communication link, reporting patient progress to physicians, the Clinical Manager, the care team, patients, and families, while ensuring all required evaluations, plans, progress notes, and discharge summaries are submitted promptly and thoroughly.
