This position invites you to evaluate, design, and implement comprehensive plans of care while actively participating in local community service. Each Plan of Care is crafted to increase, maintain, or restore function, minimize or prevent disability, and assist patients in reaching their maximum performance levels.
You will manage the full patient cycle, from initial evaluation and treatment planning to progress monitoring, physician communication, discharge, and follow-up. A key aspect of this role involves accompanying patients during physician visits and establishing professional relationships with referral sources, including physicians, PAs, ARNPs, and case managers.
Beyond clinical duties, you will mentor new clinic staff and contribute to the community by hosting booths, presenting in-services, providing coverage at special events, and performing complimentary screenings. We are also pioneering the use of Generative AI Documentation Scribes to handle compliant clinical documentation in real time, ensuring you can stay fully present with your patients.
