As a Home Health Physical Therapist Assistant at CenterWell Home Health, you will become a vital part of our caring community. Reporting to the Clinical Manager, your mission is to help patients regain strength, mobility, and independence from the comfort of their own homes. By delivering personalized care that focuses on functional improvement, you will empower individuals to overcome limitations and enjoy a better quality of life.
In this role, you will administer physical therapy treatment plans developed by the Physical Therapist, working onsite in patients' homes. Your day-to-day involves delivering non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and various modalities such as heat, light, cold, water, and electrical treatments. You will also be responsible for maintaining accurate clinical documentation, attending educational meetings, and coordinating department in-services. Collaboration is key, as you will work closely with a team of healthcare professionals and promptly notify the Physical Therapist or Branch Director of any equipment failures, supply shortages, or changes in staff or client status.
This role is patient-facing and is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to undergo TB screening. Additionally, as this position requires travel to patient homes, it is part of Humana's driver safety program. Candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with state minimums or specific coverage limits.
